A low value of MCV can indicate RBCs are smaller than normal (microcytic), this can be related to iron deficiency anemia or thalassemias, etc.

A high value of MCV can indicate RBCs are larger than normal (macrocytic), for example in anemia caused by vitamin B12 or folate deficiency, myelodysplasia, liver disease, hypothyroidism
